RECOMMENDATIONS:

APPROVE AND AUTHORIZE the Purchasing Agent, or designee, to purchase on behalf of the Health Services Director, up to 1,140 AC Transit passes each with a $2.50 value totaling $2,850; 1,296 Tri-Delta passes each with a $2.00 value totaling $2,592; 170 Tri-Delta Paratransit passes each with a $2.75 value totaling $467.50; 1,080 County Connection passes each with a $2.50 value totaling $2,700; 36 County Connection LINK passes each with a $5.00 value totaling $180; 220 East Bay Paratransit passes each with a $5.00 value totaling $1,100; 120 West Cat passes each with a $5.00 value totaling $600; and up to $500 in shipping costs for a total amount not to exceed $10,990 to be provided to low-income clients attending medical, social, and support service appointments served through Enhanced Care Management.

 

FISCAL IMPACT:

The $10,990 expenditure will be funded by the Future of Public Health Funding (FoPH, Cost Center #5898). There is no impact to the County General Fund.  

 

BACKGROUND:

Enhanced Care Management (ECM) is person-centered care management provided to the highest-need Medi-Cal enrollees, primarily through in-person engagement where beneficiaries live, seek care, and choose to access services. ECM is part of the CalAIM initiative set forth by the California Department of Health Care Services. Many community members being served by ECM rely on public transportation to make it to and from social needs appointments. The provision of non-emergency transportation services through vouchers enables clients to access or be retained in core medical and support services. All vouchers are distributed based on need.

 

CONSEQUENCE OF NEGATIVE ACTION:

If this action is not approved, the clientele supported by the ECM team will risk being unable to travel to vital medical appointments and support services.
