RECOMMENDATIONS:
APPROVE and AUTHORIZE the Employment and Human Services Director, or designee, to execute a contract amendment with Meals on Wheels Diablo Region, effective October 1, 2025, with no change to the payment limit of $245,676 for the provision of the Dignity at Home Fall Prevention Program services for the period October 1, 2024, through September 30, 2026.
FISCAL IMPACT:
100% County Measure X Funds; $92,128.50 of which was budgeted in FY 2024-2025; $122,838.00 of which is budgeted in FY 2025-2026; and $30,709.50 of which will be budgeted in FY 2026-2027.
BACKGROUND:
On November 5, 2024, the Board approved Consent Item C.28. authorizing a contract with Meals on Wheels Diablo Region, in an amount not to exceed $245,676 for the provision of Dignity at Home Fall Prevention (DHFP) Program services for the period October 1, 2024, through September 30, 2026.
This contract amendment will allow Meals on Wheels Diablo Region to provide the DHFP Program services for older adults aged 60+ and adults with disabilities aged 18+ living in Contra Costa County who are deemed at risk of falling by providing fall and injury prevention information, education, home repair/modifications, assistive devices, and other supports in order for eligible individuals receive the necessary help they need to remain independent and safe at home with no change to the payment limit of $245,676 and no change to the term October 1, 2024, through September 30, 2026.
CONSEQUENCE OF NEGATIVE ACTION:
Should the proposed action not be approved by the Board of Supervisors, Contractor will not provide DHFP services or home repair/modification services to eligible residents of Contra Costa County.

The services provided under this Contract support one (1) of the five (5) Contra Costa County’s community outcomes: (4) “Families that are Safe, Stable and Nurturing.”
