RECOMMENDATIONS:
APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Contract Amendment #25-071-16 with Public Health Foundation Enterprises, Inc. (dba Heluna Health), a non-profit corporation, effective May 1, 2026, to amend Contract #25-071-15, to decrease the payment limit by $2,460,027, from $11,943,357 to a new payment limit of $9,483,330 to reduce staffing and transitional living assistance services with no change in the term ending June 30, 2026.
FISCAL IMPACT:
Approval of this Contract Amendment will result in decreased budgeted expenditures of $2,460,027 funded by 13% Federal Grant Funds, 81% State Grant Funds, 4% County General Fund, and 2% Local Grants Funds.
BACKGROUND:
This Contract meets the social needs of County’s population by providing shelter, transitional and permanent supportive housing program for homeless youth and adults in Contra Costa County. Public Health Foundation Enterprise, Inc. has been providing services to the County under this Contract since April 1, 2015. This Contractor has a long-standing relationship with Contra Costa County and has routinely demonstrated specialized expertise in providing full scope Human Resources to employees operating under Health, Housing and Homeless Services. This includes services provided to Emergency Shelters, Transitional Housing Programs, CE, outreach provided to individuals and families experiencing homelessness. The Contractor understands the needs of the County and has knowledge and experience with the complex nature of what it takes to staff street outreach services with individuals sufficient to meet contractual obligations and ensure appropriate staff to client ratios.
This Contract is entered into under and subject to the following legal authorities: California Government Code §§ 26227 and 31000. This Contract was approved by Health Services Personnel to ensure there is no conflict with labor relations. The Health, Housing and Homeless Services Division Contract Monitoring staff meet on a regular basis to ensure monitoring of performance measures set forth in the Contract are upheld. This Contractor was approved by the Public Works Department’s Purchasing Division on June 5, 2024.
On December 9, 2025, the Board of Supervisors approved Contract #25-071-15 with Public Health Foundation Enterprise, Inc. (dba Heluna Health), in an amount not to exceed $11,943,357 to provide CE, emergency shelter, and transitional and permanent supportive housing for homeless youth and adults in Contra Costa County for the period July 1, 2025 through June 30, 2026.
Approval of Contract Amendment 25-071-16 will allow the Contractor to reduce staffing and continue to provide emergency shelter, CE, and permanent supportive housing services through June 30, 2026. This Amendment reflects a reduction in Contractor staffing after the Division converted seventeen (17) full-time equivalent (FTE)s from contracted positions to County positions and discontinued Contractor provided services for (1) youth transitional living assistance program.
CONSEQUENCE OF NEGATIVE ACTION:
If this Contract Amendment is not approved, Contractor will have an excess of funds that could otherwise be reallocated where needed.
CHILDREN’S IMPACT STATEMENT:
This program supports the following Board of Supervisors’ community outcomes: “Families that are Safe, Stable, and Nurturing”; and “Communities that are Safe and Provide a High Quality of Life for Children and Families”. Expected program outcomes include an increase in positive social and emotional development as measured by the Child and Adolescent Functional Assessment Scale (CAFAS).
