Report Title: Contract #23-573-11 with Center for Human Development

RECOMMENDATIONS:
APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Contract #23-573-11 with Center for Human Development, a non-profit corporation, in an amount not to exceed $887,690 to provide community outreach, care coordination and Medi-Cal Navigator’s project services for the period July 1, 2024 through June 30, 2025.
FISCAL IMPACT:
Approval of this contract will result in annual contractual service expenditures of up to $887,690 and will be funded by 25% Medi-Cal Administrative Activities and 75% Reducing Health Disparities Grant.
BACKGROUND:
The County has been contracting with Center for Human Development since July 1, 2015 providing community outreach, care coordination, Medi-Cal Navigator’s project services, jail reentry support services and health education to patients and residents of Contra Costa County. This contract is entered into under and subject to the following legal authorities: California Government Code §§ 26227 and 31000. Health Services Personnel approved this contract to ensure no conflicts with labor relations. Public Health Quality Management, Utilization Management and contract monitor staff meet on a regular basis to ensure monitoring and performance measures in the contract are upheld. This contractor has been approved by the Public Works Department’s Purchasing Division on May 21, 2024.
On June 27, 2023, the Board of Supervisors approved Contract #23-573-9 with Center for Human Development, in an amount not to exceed $868,907 for the provision of community outreach, care coordination and Medi-Cal Navigator, for the period July 1, 2023 through June 30, 2024.
On June 4, 2024, the Board of Supervisors approved Amendment Agreement #23-573-10 with Center for Human Development, to increase the payment limit by $50,000, from $868,907 to $918,907 for additional community outreach, care coordination and Medi-Cal Navigator’s project services, with no change in the term.
Approval of Contract #23-573-11 will allow the contractor to continue providing community outreach, care coordination and Medi-Cal Navigator’s project services, through June 30, 2025.
CONSEQUENCE OF NEGATIVE ACTION:
If this contract is not approved, vulnerable residents of Contra Costa County will not have access to this contractor’s services.
