Report Title: Approve 2027 Rate Renewal for Contra Costa Health Plans
Recommendation: CONSIDER authorizing the County Administrator, or designee, to approve premiums for insurance with the Contra Costa Health Plan (CCHP) for the period of January 1, 2027, through December 31, 2027. RECOMMENDATIONS:
AUTHORIZE the County Administrator, or Designee, to approve premiums for insurance with the Contra Costa Health Plan (CCHP) for the period of January 1, 2027 through December 31, 2027.
FISCAL IMPACT:
The revised CCHP plan premiums reflect an increase of 73.03% for Plan A, and 39.51% for Plan B, reduced from 86.83% and 50.64% respectively, as presented on September 8, 2026.
The 2027 CCHP projected total premium cost for active employees is adjusted from approximately $70.1 million to $68.6 million; the projected total premium CCHP cost for 2026 is expected to reach $33.5 million by year’s end. The currently negotiated cost to the County is adjusted from approximately $16 million to $13.6 million of that total ($31 million in 2026).
BACKGROUND:
On September 8, 2026, the 2027 plan year health rates were presented to the Board of Supervisors for all health plans. The premiums for CCHP on that agenda reflected an increase of 86.83% for CCHP Plan A and 50.64% for CCHP Plan B. The Board did not take action on the CCHP premiums on September 8 because updated premiums were provided by the Contra Costa Health Plan which reflected a decrease in the rates previously presented for CCHP Plan A and Plan B but instead reflect a premium increase of 73.03% for CCHP Plan A and 39.51% for CCHP Plan B.
The updated “NonPERS Renewal Rates BOS Attachment 1 Final” chart displays the revised Two Tier and Three Tier premiums, and the revised “Draft Rate Sheets Attachment 2” reflects the revisions mentioned above.
CONSEQUENCE OF NEGATIVE ACTION:
To prevent the disruption of services for group benefits that are offered to eligible active employees, retirees, survivors and dependents, it is necessary to accept the revised CCHP premium renewals prior to open enrollment which is planned for October 12, 2026 - October 30, 2026 for the 2027 plan year.
