RECOMMENDATIONS:

APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Grant Agreement #28-986-2, with the California Department of Developmental Services (DDS), to extend the term of Grant Agreement #28-986-1 from April 30, 2024 to June 15, 2024 with no change in the amount payable to County of $317,000 to continue providing specialty health ambassadors to promote equity and reduce disparities for persons with developmental disabilities.

 

FISCAL IMPACT:

Approval of this Grant Agreement will not impact the original funding to the County of up to $317,000 from the DDS.  (No County match is required)

 

BACKGROUND:

DDS is authorized by the California Welfare & Institutions Code to award grants to community-based organizations to promote equity and reduce disparities for persons with developmental disabilities in California.  The Specialty Health Ambassador Program will provide services to decrease health disparities for persons with developmental disabilities in Contra Costa County through strategic outreach; community education including presentations to local agencies and coalitions; engagement events in the communities such as communities, schools and places of worship; and create and distribute outreach materials to address gaps in existing services.

 

On March 21, 2023, the Board of Supervisors approved Grant Agreement #28-986-1 with DDS, in an amount not to exceed $317,000 to provide specialty health ambassadors for the period May 1, 2023 through April 30, 2024. This Grant Agreement included agreement to indemnify, defend and hold harmless the State from any claims arising out of, or in connection with, the performance of this agreement.

Approval of Grant Agreement #28-986-2 will allow the County to continue the specialty health ambassador program through June 15, 2024. 

 

CONSEQUENCE OF NEGATIVE ACTION:

If this grant agreement is not approved, grant termination date will not be extended through June 15, 2024 and strategic outreach services will end on April 30, 2024.
