Report Title: Agreement #29-812-10 with Concord/Pleasant Hill Health Care District, a subsidiary of the City of Concord
Recommendation: Recommendation of the County Administrator

RECOMMENDATIONS:
APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Agreement #29-812-10 with Concord/Pleasant Hill Health Care District, to pay the County an amount not to exceed $13,000, for County to provide homeless outreach services under the Coordinated Outreach, Referral and Engagement (CORE) Program, for the period from July 1, 2026 through June 30, 2027.
FISCAL IMPACT:
Approval of this Agreement will allow the County to receive an amount not to exceed $13,000 from the Concord/Pleasant Hill Health Care District to provide homeless outreach services. No County match required.
BACKGROUND:
The CORE Program identifies and initiates contact with homeless individuals living on the streets, assesses their housing and service needs, and facilitates connections to shelter, benefits, behavioral health and primary healthcare services throughout Contra Costa County. CORE teams serve as an entry point to County’s Coordinated Entry System for unsheltered persons and work to locate, engage, stabilize and house chronically homeless individuals and families. County has been contracting with Concord/Pleasant Hill Health Care District since August 2017 to provide homeless outreach services.
On September 9, 2025, the Board of Supervisors approved Agreement #29-812-9 to receive funds in an amount not to exceed $13,000 from the Concord/Pleasant Hill Health Care District for the provision of homeless outreach services under the CORE Program for the period from July 1, 2025 through June 30, 2026.
Approval of Agreement #29-812-10 will allow County to continue to receive funds to provide homeless outreach services under the CORE Program through June 30, 2027. This Agreement includes an indemnity provision that requires the County to defend, indemnify, save, and hold harmless the District for any claims arising out of County’s performance under this Agreement. This Agreement is late due to County not receiving the finalized documents from Concord/Pleasant Hill Health Care District until August 10, 2026.
CONSEQUENCE OF NEGATIVE ACTION:
If this Agreement is not approved, County will not receive funding and without such funding, the CORE program may have to operate at a reduced capacity.
