Report Title: Contract Amendment/Extension #25-117-2 with Contra Costa Interfaith Transitional Housing, Inc. (dba Hope Solutions)

RECOMMENDATIONS:
APPROVE and AUTHORIZE the Interim Health Services Director, or designee, to execute on behalf of the County Contract Amendment/Extension #25-117-2 with Contra Costa Interfaith Transitional Housing, Inc. (dba Hope Solutions), a non-profit corporation, effective June 1, 2025, to increase the payment limit by $206,000, from $395,000 to a new payment limit of $601,000 and to extend the termination from June 30, 2025 to June 30, 2026 for additional housing support services to residents in Continuum of Care (CoC) Permanent Supportive Housing (PSH) programs in Contra Costa County.
FISCAL IMPACT:
Approval of this Contract Amendment/Extension will result in additional budgeted of up to $206,000 as funded 100% by Measure X funds. (No rate increase)
BACKGROUND:
This Contract meets the social needs of County’s population by providing housing support services to Contra Costa County homeless individuals on probation, which includes including financial planning, life skills, food shopping, food assistance, substance abuse treatment, education and training, employment and benefits assistance, health care, mental health counseling, childcare, and credit repair. Contractor has been providing supportive housing services since July 2023. This Contractor was selected for services due to a history of positive housing outcomes, operational efficiency, and cost-effectiveness which makes them the most reliable choice to ensure project success.
This Contract is entered into under and subject to the following legal authorities: California Government Code § 26227 and 31000. This Contract was approved by Health Services Personnel to ensure there is no conflict with labor relations. The Health, Housing and Homeless Services Division Contract Monitoring staff meet on a regular basis to ensure monitoring of performance measures set forth in the contract are upheld. This Contractor was approved by the Public Works Department’s Purchasing Division on August 23, 2023.
On August 13, 2024, the Board of Supervisors approved Contract #25-117-1 with Contra Costa Interfaith Transitional Housing, Inc. (dba Hope Solutions), in an amount not to exceed $395,000 for the provision of housing support services to residents in CoC PSH programs for the period July 1, 2024 through June 30, 2025.
Approval of Contract Amendment/Extension #25-117-2 will allow the Contractor to continue providing housing support services through June 30, 2026.
CONSEQUENCE OF NEGATIVE ACTION:
If this Contract Amendment/Extension is not approved, individuals needing supportive housing will not have access to Contractor’s services and may remain or become homeless.
