2026 LMHA Program Grant Administration Update
LMHA Program achieved a high rate (90%) of timely mid-year report submissions from LMHA contractors. Ongoing technical assistance, regular communication, and clear reporting guidance contributed to improved compliance and provided valuable data to monitor program progress and outcomes. Below are a few examples of LMHA-funded programs that have made a significant difference in improving health outcomes and making an overall impact on communities.
LMHA Health Priorities: Partner Funded Outcome and Impact Report (6-months)
Healthcare Access
St. Vincent de Paul Free Medical Clinic provided 372 uninsured patients with 574 free medical visits and 1,770 free ancillary services such as labs, diagnostics, prescriptions, and medical supplies.
United Latino Voices (ULV) assisted 148 Bay Point residents with Medi-Cal and Basic Health Plan enrollment, renewals, and healthcare navigation, improving access to care for immigrant families.
Opportunity Junction’s Health Career Pathway Program trained Certified Nursing Assistants (CNA) and Medical Assistants (MA), helping 64 participants gain the skills needed to enter high‑demand healthcare jobs, thereby expanding the local healthcare workforce and improving community access to qualified providers.
Hijas del Campo’s Community Health Worker supported farmworker families with Medi‑Cal, Contra Costa Basic Health Plan, and CalFresh enrollment and renewals, improving access to health insurance and preventing lapses in coverage.
Mental Health
Family Justice Center - TeleHealth supported survivors with access to trauma and psychiatric telehealth care, including emergency evaluations and follow-up support.
Monument Impact - Mentes Positivas en Acción (MPA) - Delivered 8 weeks of culturally responsive, trauma‑informed mental health group sessions in Spanish, helping Latino participants build emotional regulation, mindfulness, coping skills, problem‑solving abilities, and resilience.
Cypress Resilience Project delivered skills‑based mental health trainings (Mental Health First Aid, QPR suicide‑prevention, A.I.D.), equipping 48 participants from local community organizations to recognize early signs of mental distress and respond safely and effectively.
Full Circle of Choices - BE WELL Initiative provided bilingual, peer‑led mental wellness education to 32 community members and distributed 27 bilingual Mental Wellness Kits with regulation tools and personalized action plans, strengthening participants’ coping skills and emotional resilience
Chronic Disease
St. Vincent de Paul RotaCare Free Clinic delivered diabetes, hypertension, and asthma education to 98 patients and provided free diagnostic and treatment supplies (e.g., glucometers).
Healthy Hearts Institute distributed 675 lbs of fresh produce and provided nutrition education, helping families improve diet quality and reduce chronic disease risk.
The Bay Compassion - Compassion Health Initiative launched the Healthy Life Food Pantry, a 12‑week “Food Is Medicine” program for individuals with diabetes and chronic conditions, providing weekly nutrition classes, healthy meals, and take‑home groceries focused on blood‑sugar control.
Unhoused Support
The Bay Compassion Clean Start Homeless Outreach provided 1,056 hot showers to unhoused residents in Bay Point and Pittsburg, plus 390 sets of clean clothing, hygiene supplies, food, and care navigation - serving 206 unique individuals experiencing homelessness.
Substance Use
Ujima Family Recovery Services - Youth Substance Use Prevention Program Conducted 84 outreach activities, strengthening referral pathways with schools, youth-serving organizations, and behavioral health providers to identify adolescents showing early signs of risky substance use.
Reported LMHA-Funded Challenges: Several organizations noted broad challenges adapting to new federal requirements for Medicaid and changes in reporting and compliance expectations, which created added workload during a period of limited staff capacity. Agencies facing staff shortages found it difficult to balance everyday operations with new Medicaid administrative demands, which in turn affected their ability to maintain consistent outreach and gather reliable data. Many programs also indicated a need for clearer guidance and support to navigate these changes, particularly as they worked with partners experiencing similar constraints. Overall, the reports reflect a sector managing evolving expectations while contending with resource limitations and operational barriers. While most contractors submitted reports on time, 3 organizations did not submit their mid-year report by the deadline, requiring additional follow-up and technical assistance to obtain the required information and ensure compliance with grant reporting requirements.

CCH staff continue to provide one-on-one technical assistance with invoice preparation and the required supporting documentation.
LMHA Grantee Engagement and Garden Updates
In June and July, CCH staff distributed a monthly LMHA Community Newsletter for sharing upcoming community resources and local networking opportunities for the LMHA contractors. Additionally, the August Los Medanos Health Area Connection Newsletter, distributed quarterly, continues to keep contractors updated with upcoming reminders, local community events, and spotlights LMHA-funded programs to showcase their work within the community.
The Los Medanos Community Garden continues to run smoothly and foster a collaborative, welcoming environment. This season, gardeners participated in two successful community workdays focused on cleaning and maintaining shared spaces. UC Master Gardeners have been hosting monthly educational workshops every fourth Saturday from April through September, offering hands-on learning opportunities open to the public. While the garden has experienced some minor theft, leadership has taken steps to improve safety, including removing an old deck due to rotting wood and potential hazards. Overall, the garden remains active, engaged, and committed to maintaining a safe and supportive space for all gardeners.
SAVE THE DATE! The Los Medanos Health Area Grantee Poster Showcase is tentatively scheduled for October 15, 2026, from 10 am-2 pm. The goal of the showcase is to highlight LMHA grantee programs, share program outcomes, foster collaboration, and demonstrate the community impact of LMHA’s grant-funded projects. The Los Medanos Community Garden will also be open for a guided tour to learn more about its programs, features, and community impact while exploring how the space promotes health, education, and community engagement.
