Information

TCAB will review concentrated disparities and priority service needs across Contra Costa County, particularly impacting Black communities. This information is intended to guide RFP design and inform approach to aligning County services with community-based provider investments within the Network model.

Presentations were intended to inform TCAB decision-making regarding funding priorities, service alignment, and network design, with a focus on where investments can drive improved outcomes rather than duplicating existing services.

Referral History and Update

At the 4/13/26 meeting, the TCAB received presentations from Contra Costa Health Services and the Employment and Human Services Department on existing programs and services aligned with the Federal D. Glover Community Wellness Network’s priority service areas:

                     Behavioral Health

                     Housing Navigation

                     Preventative Health Care

                     Infant and Maternal Health

                     Resource Navigation

                     Reentry Support

Each presentation provided:

                     Overview of services and program models

                     Populations served, including reach within Black/African American communities

                     Data on utilization, outcomes, and disparities

                     Identified gaps and unmet needs

                     Opportunities for targeted investment and partnership

                     Receive and engage in discussion

                     Identify gaps, overlaps, and opportunities for community-based investment

                     Provide direction to inform RFP design, including service categories, funding priorities, and partnership expectations in ad hoc committees as assigned
