Report Title:                      Contra Costa Health Plan Quality Improvement and Health Equity Activities Reports (January - June 2026)

Recommendation: Recommendation of the County Administrator

RECOMMENDATIONS:

ACCEPT the Contra Costa Health Plan (CCHP) quarterly Quality Improvement and Health Equity Committee (QIHEC) Activities Reports for Quarter 1 (Q1) and Quarter 2 (Q2) of calendar year 2026, in accordance with Department of Health Care Services (DHCS) contract requirements and as recommended by the CCHP Joint Conference Committee (JCC).

FISCAL IMPACT:

There is no fiscal impact for this action.

BACKGROUND:

CCHP is required to implement and maintain a Quality Improvement and Health Equity Committee (QIHEC). The QIHEC is led jointly by CCHP’s Medical Director (or designee) and CCHP’s Chief Health Equity Officer and must include participation from a broad range of network providers. The QIHEC is responsible for directing and overseeing all Quality Improvement and Health Equity program activities.

CCHP is required to submit a written summary of QIHEC activities to its Governing Board and DHCS each quarter and make these reports publicly available on its website.  Although the CCHP JCC accepted the Quality and Health Equity Report at its June 5 meeting, the published QIHEC activities report for the first quarter of 2026 was omitted from the agenda packet. To address that omission and provide the current quarterly update, this report includes both Attachment A (QIHEC Q1 2026 Quarterly Activities Report) and Attachment B (QIHEC Q2 2026 Quarterly Activities Report).

SUMMARY OF FIRST QUARTER OF 2026 REPORT

During the first quarter of 2026, CCHP moved forward on key work across accreditation, quality improvement, population health and regulatory compliance. CCHP reviewed and completed major program documents. Policies were also updated and approved. CCHP received updates on National Committee for Quality Assurance (NCQA) Accreditation renewal. And the health plan kept an eye on ongoing performance improvement initiatives, member experience survey activities and preventive outreach. CCHP focused on strengthening access to care, addressing disparities and improving infrastructure according to state and national standards.

CCHP completed the NCQA Health Plan Accreditation audit and was successful in achieving accreditation. CCHP improved Healthcare Effectiveness Data and Information Set (HEDIS) production processes and got strong results from external audits. Performance Improvement Projects showed meaningful outreach for cervical cancer screening and well-care visit. Population health activities have expanded community engagement and member education. CCHP continues to talk with providers, monitor patient safety and conduct facility reviews. All this work supports ongoing quality oversight and network performance.

SUMMARY OF SECOND QUARTER OF 2026 REPORT

CCHP submitted its Measurement Year (MY) 2025 Healthcare Effectiveness Data and Information Set (HEDIS) in June 2026. These measures are used by a variety of regulators for enforcement purposes. CCHP anticipates earning back 100% of capitation dollars for CY 2025 through the DHCS Quality Withhold program. CCHP surpassed the DHCS Minimum Performance Level (MPL) for all Medi-Cal Accountability Set (MCAS) measures in 2025. CCHP will not receive any sanctions.

CCHP successfully met all HEDIS requirements and exceeded Minimum Performance Levels in all measures. CCHP also submitted results ahead of National Committee for Quality Assurance (NCQA) deadlines. Outreach and Performance Improvement Projects continued to show progress. This includes increased well‑care visit and cervical cancer screening rates. Child health equity work continued through targeted mailings and community events. Population health efforts included the following:

•                     Production of health education newsletters;

•                     Participation in countywide planning committees; and

•                     Launch of an Advice Nurse awareness initiative.

Patient safety oversight continued through investigations, site reviews and promotion of timely access standards. CCHP also strengthened provider engagement. This was achieved through quarterly training, newsletters and nine targeted quality meetings with network partners.

CONSEQUENCE OF NEGATIVE ACTION:

Failure to accept the QIHEC Quarterly Reports would prevent CCHP from meeting its contractual obligation with DHCS and impact CCHP’s compliance standing.
