RECOMMENDATIONS:
APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Contract #25-136 with Contra Costa Interfaith Transitional Housing, Inc. (dba Hope Solutions), a non-profit corporation, in an amount not to exceed $1,977,733 to provide rapid rehousing, housing support and property management services for homeless Transitional Age Youth (TAY) ) in Contra Costa County, and to provide 116 gifts cards with a total value of $6,040 (14 Amazon gift cards up to $100 per card, 13 Target gift cards up to $50 per card, 14 Walmart gift cards up to $25 per card, 52 Uber gift cards up to $50 per card, 12 Clipper Cards up to $50 per card, and 11 West Contra Costa Transit (WestCAT) gift cards up to $40 per card) for TAY participating in the Transitional Living Program for the period October 1, 2025 through September 30, 2027.
FISCAL IMPACT:
Approval of this Contract will result in annual expenditures up to $1,977,733 as budgeted by the department for FY’s 2025-27 and will be funded by 52% Homeless Housing Assistance and Prevention (HHAP), 25% Federal funds, 7% Employment and Human Services Department (EHSD), 6% Mental Health Services Act (MHSA), 5% Probation Department Funds and 5% Measure X Housing funds.
BACKGROUND:
This Contract meets the social needs of the County’s population by providing rapid rehousing, housing support and property management services for homeless TAY on probation, which includes case management and housing navigation services. This Contractor was selected to provide services due to a history of positive housing outcomes, operational efficiency, and cost-effectiveness which makes them the most reliable choice to ensure project success. This Contractor has fostered a deep understanding of the County’s organizations, mission, values, and long-term objectives starting October 1, 2025.
This Contract is entered into under and subject to the following legal authorities: California Government Code § 26227 and 31000. This Contract was approved by Health Services Personnel to ensure there is no conflict with labor relations. The Health, Housing and Homeless (H3) Services Division Contract Monitoring staff meet on a regular basis to ensure monitoring of performance measures set forth in the contract are upheld. This Contractor was approved by the Public Works Department’s Purchasing Division on August 13, 2025.
This Contract will allow Contractor to purchase and provide gift card incentives for TAY receiving rapid rehousing and housing support services to be used for interdependent living skill achievements, successfully completing program activities and accomplishing program goals including obtaining a General Education (GED) or High School Diploma, increasing job skills, obtaining and maintaining employment, rebuilding credit scores, and securing housing. Gift cards are funded 100% by Homeless Housing Assistance and Prevention (HHAP) grant funds.
Approval of Contract #25-136 will allow the Contractor to provide rapid rehousing, housing support and property management services for homeless TAY through September 30, 2027. This Contract is late due to prolonged negotiations between the Contractor and H3 Division.
CONSEQUENCE OF NEGATIVE ACTION:
If this Contract is not approved, homeless adults and TAY on probation will not have access to Contractor’s services and may remain or become homeless.
CHILDREN’S IMPACT STATEMENT:
This program supports the following Board of Supervisors’ community outcomes: “Children Ready for and Succeeding in School”; “Families that are Safe, Stable, and Nurturing”; and “Communities that are Safe and Provide a High Quality of Life for Children and Families”. Expected program outcomes include an increase in positive social and emotional development as measured by the Child and Adolescent Functional Assessment Scale (CAFAS).
