Report Title: Contract Choice In Aging for Adult Protection Services Case Management Services
Recommendation: Recommendation of the County Administrator

RECOMMENDATIONS:
APPROVE and AUTHORIZE the Employment and Human Services Director, or designee, to execute a contract with Choice In Aging, in an amount not to exceed $347,380 to provide Adult Protective Services case management services for older adults for the period July 1, 2026, through June 30, 2028.
FISCAL IMPACT:
100% Measure X Funds of which $173,690 is budgeted in FY 26-27 and $173,690 will be budgeted in FY 27-28.
BACKGROUND:
The Employment and Human Services Department seeks to contract with Choice In Aging for countywide case management services with special attention to addressing priority populations, as defined in the Older Americans Act. This Contractor was selected through Contra Costa County’s procurement process, Request for Proposals #1235, for supportive services, Adult Protective Services (APS) case management, and Measure X to provide services for older persons aged 60 and above, regardless of income, and residing in Contra Costa County, that live in non-congregate settings and not in long-term care facilities, residential care facilities, state development centers, state hospitals, and other institutionalized settings.
These services are provided through a Measure X allocation authorized on November 16, 2021 (Item D.5), for Master Plan for Aging/Local Community Based Aging Services.
This action seeks to execute a contract with Choice in Aging to provide APS case management services for the period July 1, 2026, through June 30, 2028.
CHILDREN'S IMPACT STATEMENT:
This contract supports one of the community outcomes established in the Children’s Report Card: (4) "Families that are Safe, Stable and Nurturing."
CONSEQUENCE OF NEGATIVE ACTION:
Should the proposed action not be approved by the Board of Supervisors, County will provide fewer case management services for older adults 60 years and older, and dependent adults aged 18 years and older.
