Report Title: Amendment #23-573-10 with Center for Human Development

RECOMMENDATIONS:
APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Contract Amendment #23-573-10 with Center for Human Development, a non-profit corporation, effective May 1, 2024, to amend Contract 23-573-9, to increase the payment limit by $50,000, from $868,907 to $918,907 for additional community outreach, care coordination and Medi-Cal Navigator’s project services, with no change in the original term of July 1, 2023 through June 30, 2024.
FISCAL IMPACT:
Approval of this Amendment will result in additional annual expenditures of up to $50,000 and will be funded 100% by American Rescue Plan Act funding. (No rate increase)
BACKGROUND:
The County has been contracting with Center for Human Development since July 1, 2015 providing community outreach, care coordination, Medi-Cal Navigator’s project services, jail reentry support services and health education to patients and residents of Contra Costa County. This contract is entered into under and subject to the following legal authorities: California Government Code §§ 26227 and 31000. Health Services Personnel approved this contract to ensure no conflicts with labor relations. Public Health Quality Management, Utilization Management and contract monitor staff meet on a regular basis to ensure monitoring and performance measures in the contract are upheld.
On June 27, 2023, the Board of Supervisors approved Contract #23-573-9 with Center for Human Development, in an amount not to exceed $868,907 for the provision of community outreach, care coordination and Medi-Cal Navigator, for the period July 1, 2023 through June 30, 2024.
Approval of Amendment #23-573-10 will allow the contractor to provide additional community outreach, care coordination and Medi-Cal Navigator’s project services, through June 30, 2024.
CONSEQUENCE OF NEGATIVE ACTION:
If this Amendment is not approved, vulnerable residents of Contra Costa County will not have access to the contractor’s services.
CHILDREN’S IMPACT STATEMENT:
This program supports the following Board of Supervisors’ community outcomes: “Communities that are Safe and Provide a High Quality of Life for Children and Families”. Expected program outcomes include a decrease in reentrant recidivism, a decrease in food insecurity, an increase in patient’s access to Medi-Cal program, health improvements for children with pediatric obesity, adults with diabetes and hypertension and women who are pregnant.
