RECOMMENDATIONS:

RATIFY the Health Services Director, or designee’s, January 7, 2026 execution of Grant Agreement #29-540-12 (Federal Grant #CA1071L9T052412) with the United States Department of Housing and Urban Development (HUD), to receive McKinney-Vento Act funds, payable to the County in an amount not to exceed $501,449 for the County’s Homeless Destination Home Program to provide permanent supportive housing to adults throughout Contra Costa County for the period of December 1, 2025 through November 30, 2026 as recommended by the Health Services Director.

 

FISCAL IMPACT:

Approval of this Grant Agreement will result in an amount not to exceed $501,449 in McKinney-Vento Act funds from HUD. An 8% County match of $41,298 is required.

 

BACKGROUND:

The County’s Homeless Destination Home Program (“Program”) provides permanent supportive housing to adults throughout Contra Costa County.  The Program includes case management, and assistance with enrolling in benefits and services, including access to peer support groups. These services are all part of a larger continuum of services that allow individuals to work through many of the issues that led to their homelessness, while simultaneously moving toward a more sustainable future. Other benefits of the Program include assistance in accessing benefits like Medi-Cal, Social Security Income and Supplemental Security Disability Insurance, referral to services, health and life skills education, crisis intervention and counseling. Participants enrolled in the Program will have access to Behavioral Health/Mental Health Services, County Health Care for the Homeless, and Alcohol and Other Drug Services. County has been receiving grant funds from HUD for the Program since 2013.

 

On March 25, 2025, the Board of Supervisors approved Grant Agreement #29-540-11 with HUD, for McKinney-Vento Act funds, to pay County an amount not to exceed $$451,544 for County’s Homeless Destination Home Program for the period from December 1, 2024 through November 30, 2025.

 

Approval of Grant Agreement #29-540-12 will allow the County to continue to receive funds for the Program, through November 30, 2026. This Grant Agreement is delayed due to County not receiving the Agreement document from HUD until December 15, 2025.

 

CONSEQUENCE OF NEGATIVE ACTION:

If this Contract is not ratified, the County will not receive funding in an amount up to $501,449 to support the County’s Homeless Destination Home Program.
