IN THE MATTER OF In the matter recognizing Program Manager

Heather Sweeten-Healy upon her retirement

from Contra Costa Health Services.

 

WHEREAS, in 1994 Program Manager Heather Sweeten-Healy attained a Master’s Degree in Social Work from the California State University at Fresno and began her career in both Monterey and Contra Costa Counties, where she served vulnerable residents with care, concern, empathy, and professionalism for over 32 years.

 

WHEREAS, from 1994 to 2006, Program Manager Sweeten-Healy provided field care as a Social Worker III and Psychiatric Social Worker II to chronically mentally ill adults, through crisis intervention, development, the provision of behavioral health treatment services, and mediation sessions representing County Behavioral Health with attorney and educational advocates.

 

WHEREAS, in 1998, she was awarded National Association of Social Workers, California Central Chapter, Social Worker of the Year.

 

WHEREAS, from 2002 to 2006, Program Manager Sweeten-Healy supervised managed a care clinic and led the Proposition 36 Team, where she participated in court hearings, provided collaboration with the Superior Court, the District Attorney’s Office, the Public Defender, and the Probation Department.

 

WHEREAS, in 1999, she became a Licensed Clinical Social Worker (LCSW) with the State of California Board of Behavioral Sciences.

 

WHEREAS, from 2006 to 2011, Program Manager Sweeten-Healy served the community as a Unit and Mental Health Program Supervisor. During this time, she fulfilled a three-year special assignment where she concurrently filled two full-time roles, which included, clinical supervision, supervision of two full-service partnership programs providing and expanding services to older adults with co-occurring mental health and medical diagnoses, and a temporary special assignment working out-of-class with the Drug Treatment Court, and the Proposition 36 Court.

 

WHEREAS, from 2011 to 2012, Program Manager Sweeten-Healy served the community as a Mental Health Program Supervisor and AB 3632 Coordinator, where she provided clinical supervision of the centralized Assessment Unit, providing mental health services for juveniles.

 

WHEREAS, from 2012 to 2026, Program Manager Sweeten-Healy served the community as the Program Manager of Older Adult Mental Health Services, overseeing the multi-disciplinary care team providing continuum of care to seniors throughout Contra Costa County. In this role, she provided dedicated and calm leadership of Intensive Care Management (ICM) to support the aging in-place population, provided access to collaborative treatment (IMPACT) for those experiencing depression, while receiving medical care for anxiety or post-traumatic stress disorder, and senior peer counseling to support individuals experiencing various mental health concerns.

 

NOW, THEREFORE BE IT RESOLVED that the Board of Supervisors of Contra Costa County is hereby honored to recognize the efforts of Program Manager Sweeten-Healy to the citizens of Contra Costa Health, and to Contra Costa County.
