FISCAL IMPACT:
The cost of the design-build contract is $3,718,082, to be funded by the Health Services Department Enterprise Fund (100%).
BACKGROUND:
Contra Costa Regional Medical Center (CCRMC), located at 2500 Alhambra Avenue in Martinez, is a County operated hospital that provides medical care and essential health services to Contra Costa County residents. The hospital relies on its nurse call system as a critical communication link between patients and clinical staff. The system allows patients to request assistance from their rooms and alerts nursing personnel when patients need attention. Reliable patient-to-staff communication is particularly important for patients who have limited mobility, require assistance with daily activities, or need timely medical attention. The nurse call system is therefore a vital component of the hospital’s daily operations and its ability to provide safe and responsive patient care.
The existing nurse call system has become obsolete and is no longer fully functional. In addition, replacement parts for the aging system are increasingly difficult to obtain, limiting the hospital’s ability to repair failed components and maintain reliable operation. As the system continues to deteriorate, the lack of available replacement parts increases the risk of prolonged service interruptions and creates ongoing challenges for hospital maintenance and clinical staff. The Department of Public Works, in coordination with CCRMC staff, has determined that replacement of the existing nurse call system is necessary to address these deficiencies, maintain dependable communication between patients and nursing personnel, and support the hospital’s continued delivery of safe and effective patient care.
The Upgrade Nurse Call System Project will replace and modernize the existing nurse call
equipment. The work includes replacing patient communication devices, wiring, control panels, and nurse station communication equipment, along with associated electrical, low voltage and life safety work and necessary repairs to affected finishes. The project will provide a complete, operational system designed to meet applicable California Building Code requirements. The project requires coordination among clinical staff, facility operations, the design-build team, and permitting authorities to maintain patient services while the existing system is replaced. The Project Criteria Documents establish the required performance and delivery requirements for this work.
In accordance with the County’s established capital project delivery practices, projects of this scale and complexity are typically managed by a dedicated project team and procured through a competitive solicitation process to ensure transparency, competition, and the selection of the most qualified team. The design-build delivery method is recognized for its ability to streamline project timelines, foster collaboration between design and construction professionals, and reduce the risk of claims or delays compared to traditional procurement approaches.
Two potential design-build entities (DBEs) were short-listed for the project through a competitive, qualifications based selection process. On March 17, 2026, the Board authorized the use of design-build procurement for this project and further authorized Public Works to issue a Request for Proposal (RFP) inviting shortlisted DBEs to submit proposals.
On July 23, 2026, Public Works issued a Request for Proposal (RFP) to the short-listed DBEs: BCCI Construction LLC (BCCI) and Streamline. On September 17, 2026, Public works received one proposal from Streamline. A selection committee consisting of Capital Projects Management Division, CCRMC Senior Management, CCRMC Facilities, CCRMC Plant Operations, and CCRMC Nursing Operations, assisted by Project Managers with Kitchell CEM, reviewed the proposal and presentation made by Streamline.
Following its review, the County determined that Streamline’s proposal was responsive, responsible, and offered the best value under the established evaluation criteria. Streamline’s proposal price of $3,693,082, including a $25,000 allowance for County authorized television repairs or replacement, is within the $3,840,000 project price identified in the RFP and Streamline’s proposed schedule meets the project schedule requirements.
The proposed action is consistent with the legal authority granted to counties under California Public Contract Code Section 22164, which authorizes the use of design-build procurement for public works projects in excess of $1,000,000, subject to the development of appropriate conflict-of-interest policies and compliance with all applicable statutory requirements. The Department of Public Works has prepared the necessary procurement documents and will ensure that all statutory and County policy requirements are met throughout the solicitation and contract award process.
The contractor will be required to enter into a Project Labor Agreement in connection with the construction services to be performed under the contract.
The Project is exempt from CEQA because the replacement of the nurse call system involves alterations to existing hospital communication and electrical systems with negligible or no expansion of existing use which is consistent with Section 15301(a) of the CEQA Guidelines.
Construction is expected to be completed in the second quarter of 2028.
CONSEQUENCE OF NEGATIVE ACTION:
If the Board does not approve the contract, the hospital at the CCRMC will continue to rely on its existing nurse call system, which is obsolete and not fully functional. Replacement parts for the aging system are increasingly difficult to obtain, limiting the hospital’s ability to repair failed equipment and maintain reliable patient-to-staff communication.
If the system fails and replacement parts are unavailable, affected portions of the system could remain out of service for extended periods. Patients in those areas may be unable to use their nurse call devices to request assistance, requiring hospital staff to implement alternative communication procedures and increase patient monitoring. These measures could require additional staff resources, disrupt normal nursing operations, and affect how quickly staff can respond to patients who need assistance.
Without replacement of the existing system, CCRMC will continue to face the risk of prolonged communication outages, increasing maintenance challenges, and potential impacts to patient care and safety.
