Recommendation: Recommendation of the County Administrator

 

RECOMMENDATIONS:

APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Grant Agreement #29-814-9 with the City of Concord, to pay the County an amount not to exceed $1,491,842 to provide homeless outreach services under the Coordinated Outreach, Referral and Engagement (CORE) Program, for the period July 1, 2026 through June 30, 2029.

 

FISCAL IMPACT:

Approval of this Grant Agreement will allow the County to receive funding in an amount not to exceed $1,491,842 over a 3-year period from the City of Concord. No County match is required.

 

BACKGROUND:

The CORE Program identifies and initiates contact with homeless individuals living on the streets, assesses their housing and service needs, and facilitates connections to shelter, benefits, behavioral health and primary healthcare services throughout Contra Costa County. CORE teams serve as an entry point to County’s Coordinated Entry System for unsheltered persons and work to locate, engage, stabilize and house chronically homeless individuals and families. Contractor has provided funding for the County’s CORE program since August 2017.

 

On April 28, 2025, the Board of Supervisors approved Agreement #29-814-8 with City of Concord, to pay the County an amount not to exceed $1,277,706 to provide homeless outreach services for the CORE Program, for the period July 1, 2023 through June 30, 2026.

 

Approval of Grant Agreement #29-814-9 will allow the County to continue to receive funds to provide CORE homeless outreach services for the City of Concord through June 30, 2029. This Grant Agreement includes an indemnification provision requiring the County to defend, indemnify and hold harmless the Contractor for any claims arising out of the performance of this Grant Agreement. This Agreement was delayed due to the Community Response division not receiving necessary documents from the City of Concord in a timely manner.

 

CONSEQUENCE OF NEGATIVE ACTION:

If this Grant Agreement is not approved, County will not receive funding and without such funding, the CORE program may have to operate at a reduced capacity.
