Receive and discuss a presentation from Jasmine Jones of the East Bay Community Foundation on early process insights from the first grantee cohort of the Federal D. Glover Community Wellness Network, the discussion will provide direction on areas for as related to RFP development and a final report with deeper analysis will be presented at the June TCAB meeting.

Referral History and Update

The Federal D. Glover Community Wellness Network represents a countywide investment strategy aimed at advancing community-driven solutions to improve health and wellness outcomes. The initial $1 million investment was stewarded by the East Bay Community Foundation, which served as the intermediary responsible for administering funds, supporting grantees, and coordinating implementation across community-based organizations (CBOs).

As TCAB continues to refine program categories, funding levels, and expectations for accountability and support, it is critical to ground decision-making in early implementation experience from the first cohort. This item aligns with TCAB’s role in guiding equitable investment strategies, strengthening accountability structures, and ensuring that funding decisions are informed by community-centered implementation experience.

Jasmine Jones will provide a high-level overview of early process insights from the first grantee cohort, focusing on implementation dynamics rather than final outcomes.

The presentation will include:

                     Observations on investment structure and distribution across program areas

                     Initial insights on geographic reach and priority populations served

                     Early reflections on grantee experience, including onboarding, communication, and reporting

                     Considerations related to intermediary role and contract administration

                     Preliminary identification of technical assistance and capacity needs across grantees

                     Emerging themes related to coordination across CBOs and county partners

This initial presentation is intended to surface key themes and inform TCAB discussion, rather than provide a comprehensive evaluation.

A more detailed analysis, including deeper findings on outcomes, challenges, and strategic implications, will be brought back to TCAB at the June meeting.

This item advances implementation of the Federal Glover Community Wellness Network.
