Report Title: Contract #77-655-4 with Abdul W. Wali, M.D.

RECOMMENDATIONS:
APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Contract #77-655-4 with Abdul W. Wali, M.D., an individual, in an amount not to exceed $600,000, to provide medical consultation and technical assistance services to the Contra Costa Health Plan (CCHP) Medical Management Team, with regard to policies and procedures used by Utilization Management for the period April 1, 2026 through March 31, 2029.
FISCAL IMPACT:
Approval of this Contract will result in contractual service expenditures of up to $600,000 over a 3-year period and will be funded 100% by CCHP Enterprise Fund II revenues.
BACKGROUND:
CCHP has an obligation to provide certain specialized medical consultation services for its members under the terms of their Individual and Group Health Plan membership Contracts with the County. This Contractor has been providing medical consultation services, including guidance and leadership, to the CCHP Medical Management Team with regard to CCHP’s best practices, policies and procedures used by Utilization Management staff since April 1, 2024.
Medical Consulting is essential to ensure that CCHP is meeting and following the Medi-Cal Managed Care Plan requirements from the State of California Department of Managed Health Care (DMHC) and Department of Health Care Services (DHCS). This Contract is entered into under and subject to the following legal authorities: California Government Code §§ 26227 and 31000; Health and Safety Code § 1451; and 42 CFR 432.10(a) Standards of Personnel Administration. Health Services Personnel approved this Contract to ensure no conflicts with labor relations. The nature of the consulting and technical services needed is complex and requires seamless coordination, integration and collaboration with existing programs and systems. This Contractor was approved by the Public Works Department’s Purchasing Division on March 4, 2026.
On March 19, 2024, the Board of Supervisors approved Contract #77-655 with Abdul W. Wali, M.D., in an amount not to exceed $400,000, to provide consultation and technical assistance to the CCHP Medical Management Team including providing guidance and leadership in maintaining the best practices for Utilization Management, for the period April 1, 2024 through March 31, 2025.
On November 5, 2024, the Board of Supervisors approved Contract Amendment #77-655-1 with Abdul W. Wali, M.D., effective November 1, 2024, to modify the Contract provisions to conform with the DMHC requirements and provide additional medical consultation services to the CCHP Medical Management Team with no change in the payment limit of $400,000 or term.
On March 11, 2025, the Board of Supervisors approved Contract Extension #77-655-2 with Abdul W. Wali, M.D., effective March 31, 2025, to extend the term from March 31, 2025 to March 31, 2026, for additional medical consultation services to the CCHP Medical Management Team with no change in the payment limit of $400,000.
Approval of Contract #77-655-4 will allow the Contractor to continue providing consultation and technical assistance services to the CCHP Medical Management team through March 31, 2029.
CONSEQUENCE OF NEGATIVE ACTION:
If this Contract is not approved, the CCHP Medical Management Team will not receive the benefits of compliance consultation from the Contractor and state mandated requirements will not be met.
