Report Title: Contract Amendment #25-094-8 with Contra Costa Interfaith Transitional Housing, Inc. (dba Hope Solutions)

RECOMMENDATIONS:
APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Contract Amendment #25-094-8 with Contra Costa Interfaith Transitional Housing, Inc. (dba Hope Solutions), a non-profit corporation, effective July 1, 2026, to amend Contract #25-094-5, to increase the payment limit by $1,350,000, from $3,652,516 to an amount not to exceed $5,002,516, to provide additional rapid rehousing and homeless prevention services for homeless adults and Transitional Age Youth (TAY) on probation with no change in the term ending June 30, 2027.
FISCAL IMPACT:
Approval of this Contract Amendment will result in additional expenditures of up to $1,350,000 and will be funded 100% by Probation Department funds.
BACKGROUND:
This Contract meets the social needs of the County’s population by providing rapid rehousing and homeless prevention services for homeless adults and TAY on probation. These services include case management and housing navigation services which will assist participants with food assistance, substance use treatment, education and training, employment and benefits assistance, health care, mental health counseling, childcare, and credit repair. Rapid rehousing services will include providing financial assistance for security deposits, utility deposits/payments, moving costs, hotel/motel vouchers, paid shelter costs, housing application and credit/background check fees, partial, tiered or full rental subsidies. This Contractor has been fostering a deep understanding of the County’s organization, mission, values, and long-term objectives since July 2021.
This Contract is entered into under and subject to the following legal authorities: California Government Code § 26227 and 31000. This Contract was approved by Health Services Personnel to ensure there is no conflict with labor relations. The H3 Division Contract Monitoring staff meet on a regular basis to ensure monitoring of performance measures set forth in the Contract are upheld. This Contractor was approved by the Public Works Department’s Purchasing Division on February 2, 2025.
On August 5, 2025, the Board of Supervisors approved Contract #25-094-5 with Contra Costa Interfaith Transitional Housing, Inc. (dba Hope Solutions), in an amount not to exceed $3,652,516, for the provision of rapid rehousing and homeless prevention services for homeless adults and TAY on probation for the period July 1, 2025 through June 30, 2027.
Approval of Contract Amendment #25-094-8 will allow the Contractor to provide additional rapid rehousing and homeless prevention services for homeless adults and TAY on probation through June 30, 2027. This Contract was delayed due to extended negotiations between the Contractor and Division.
CONSEQUENCE OF NEGATIVE ACTION:
If this Contract is not approved homeless adults and TAY on probation will not have access to Contractor’s rapid rehousing and homeless prevention support services.
