Report Title: Agreement #29-820-5 with the City of San Pablo

RECOMMENDATIONS:
APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Agreement #29-820-5 with the City of San Pablo, a government agency, to pay the County an amount not to exceed $340,127 for the Coordinated Outreach, Referral and Engagement (CORE) Program to provide homeless outreach services, for the period from July 1, 2023 through June 30, 2026.
FISCAL IMPACT:
Approval of this Agreement will allow the County to receive an amount not to exceed $340,127 over a three-year period from the City of San Pablo to provide homeless outreach services. No County match is required.
BACKGROUND:
The CORE Program locates and engages homeless clients throughout Contra Costa County. CORE teams serve as an entry point into the County’s coordinated entry system for unsheltered persons and work to locate, engage, stabilize and house chronically homeless individuals and families. The County has been providing these services to the City of San Pablo since December 1, 2018.
On February 26, 2019, the Board of Supervisors approved Grant Agreement #29-820 to receive funds in an amount of $60,570 from the City of San Pablo to operate the CORE Program and provide services to the City of San Pablo through November 30, 2019.
On January 14, 2020, the Board of Supervisors approved Grant Amendment #29-820-1 to receive additional funds up to $40,000 to a new total of $100,570 and extend the termination date from November 30, 2019 to November 30, 2020.
On March 30, 2021, the Board of Supervisors approved Grant Amendment #29-820-2 to receive additional funds up to $25,000 to a new total of $125,570 and extend the termination date from November 30, 2020 to November 30, 2021.
On June 7, 2022, the Board of Supervisors approved Grant Amendment #29-820-3 to receive additional funds up to $58,493 to a new total of $184,063 and extend the termination date from November 30, 2021 to June 30, 2022.
On July 26, 2022, the Board of Supervisors approved Grant Amendment #29-820-4 to receive additional funds up to $100,416 to a new total of $284,479 and extend the termination date from November 30, 2022 to June 30, 2023.
Approval of Agreement #29-820-5 will allow County to continue to receive funds through June 30, 2026. This contract includes mutual indemnification to hold harmless both parties for any claims arising out of the performance of this Contract. This Grant agreement is late due to the Division not receiving the grant document until January 8, 2024.
CONSEQUENCE OF NEGATIVE ACTION:
If this agreement is not approved, County will not receive funding and without such funding, the CORE program may have to operate at a reduced capacity.
