RECOMMENDATIONS:

APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Contract Amendment/Extension #25-098-4 with Contra Costa Interfaith Transitional Housing, Inc. (dba Hope Solutions), a non-profit corporation, effective February 1, 2026, to amend Contract #25-098-3, to increase the payment limit by $439,624, from $145,000, to an amount not to exceed $584,624, and to extend the termination date from June 30, 2026 to June 30, 2027, to add rapid rehousing services in addition to homelessness prevention services for individuals who are at risk of or currently experiencing homelessness and referred by the Employment and Human Services Department‘s (EHSD) Housing Disability Advocacy Program (HDAP).

 

FISCAL IMPACT:

Approval of this Contract Amendment/Extension will result in additional expenditures of up to $439,624 and will be funded 100% by EHSD Housing Disability Advocacy Program grant funds. (No rate increase)

 

BACKGROUND:

This Contract meets the social needs of County’s population by providing rapid rehousing and homeless prevention services to Contra Costa County individuals experiencing homelessness or at risk of homelessness. Contractor will provide these services to individuals approved by EHSD’s HDAP program.  EHSD will approve individuals who meet HDAP criteria and appear eligible to receive Social Security Income (SSI), Social Security Disability Income (SSDI), Veterans benefits, or Cash Assistance Program for Immigrants (CAPI). Contractor will house up to seventy-nine (79) homeless individuals referred by EHSD and assessed as having potential eligibility to any of the SSI/SSDI, veterans, or CAPI benefits.

 

This Contract is entered into under and subject to the following legal authorities: California Government Code § 26227 and 31000. This Contract was approved by Health Services Personnel to ensure there is no conflict with labor relations.  The Health, Housing and Homeless Services Division (H3) Contract Monitoring staff meet on a regular basis to ensure monitoring of performance measures set forth in the contract are upheld. The request for proposal (RFP) was posted on December 15, 2023 and closed on January 31, 2024. This Contractor was selected through the RFP process on May 15, 2024.

 

In May 2022, the Purchasing Services Manager approved and executed Contract #25-098-3 with Contra Costa Interfaith Transitional Housing, Inc. (dba Hope Solutions), in an amount not to exceed $145,000, to provide rapid re-housing and homelessness prevention services to individuals who are experiencing homelessness or are at risk for homelessness and referred by EHSD’s HDAP, for the period from October 1, 2025 through June 30, 2026.

 

Approval of Contract Amendment/Extension #25-098-4 will allow the Contractor to add rapid rehousing services in addition to homelessness prevention services for individuals who are at risk of or currently experiencing homelessness and referred by EHSD through June 30, 2027. This Contract delay is due to the Division not receiving the Interagency Agreement for the HDAP grant allocation until April 6, 2026.

 

CONSEQUENCE OF NEGATIVE ACTION:

If this Contract Amendment/Extension is not approved, homeless individuals will not have access to Contractor’s rapid rehousing and homelessness prevention services.
