RECOMMENDATIONS:
APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Contract #26-410-5 with La Clinica de La Raza, Inc., a non-profit corporation, to provide transfer and coordination of care for patients requiring emergency medical care at Contra Costa Regional Medical Center (CCRMC) for the period March 1, 2026 through February 29, 2028.
FISCAL IMPACT:
This is a non-financial agreement.
BACKGROUND:
The goal of this Care Transition Agreement is to provide continuity of care for La Clinica de La Raza, Inc. patients who are treated at CCRMC and ensure that these patients are connected with their primary medical home after a hospital visit. The County has been contracting with La Clinica de La Raza, Inc. to provide transfer and coordination of care services for patients requiring emergency medical care at CCRMC, while fostering a deep understanding of the organization's mission, values and long-term objectives since March 2016.
This Contract is entered into under and subject to the following legal authorities: California Government Code ?? 26227 and 31000. Health Services Personnel approved this Contract to ensure no conflicts with labor relations. Contractor will cooperate and participate in the County's Quality Management Program which consists of quality improvement activities to improve the quality of care, services and member experience. Cooperation includes collection and evaluation of performance measurement data and participation in the organization's clinical and service measure Quality Improvement Programs. This Contract is exempt from the solicitation requirements of Admin Bulletin 600.3, as this is a non-financial agreement.
