RECOMMENDATIONS:
APPROVE and AUTHORIZE the Interim Health Services Director, or designee, to execute on behalf of the County Contract Amendment/Extension #25-075-11 with Trinity Center Walnut Creek, a non-profit corporation, effective June 30, 2025, to amend Contract #25-075-9 (as Amended by Contract Amendment/Extension #25-075-10), to increase the payment limit by $200,000, from $400,000 to a new payment limit of $600,000, and extend the termination date from June 30, 2025 to June 30, 2026 for additional housing focused case management services to adults experiencing homelessness in Contra Costa County.

FISCAL IMPACT:
Approval of this Contract Amendment/Extension will result in additional expenditures of up to $200,000 budgeted by the department for FY 2025-26 and funded 100% by U.S. Department Housing and Urban Development (HUD) grant funds.

BACKGROUND:
This Contractor operates the Coordinated Assessment and Resource (CARE) Center and provides housing focused case management services to individuals experiencing homelessness, including financial planning, benefits and employment assistance, reporting, training, managing a household and shopping for food, substance use treatment, education and training, employment assistance, benefits assistance, health care, mental health counseling, childcare, and credit repair from community-based agencies and meeting attendance, in support of the Continuum of Care (CoC) Project for County's Coordinated Entry System (CES).
