RECOMMENDATIONS:
APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Agreement #29-604-10 with the City of Walnut Creek, to pay the County an amount not to exceed $23,065 for the operation of the Coordinated Outreach, Referral and Engagement (CORE) Program to provide homeless outreach services on nights and weekends, for the period from July 1, 2025 through June 30, 2026.
FISCAL IMPACT:
Approval of this Agreement will allow the County to receive funds in an amount not to exceed $23,065 from City of Walnut Creek's Community Development Block Grant (CDBG) and Homeless Services Fund (HSF) for Fiscal Year 2025-26. No County match is required.
BACKGROUND:
The CORE Program will serve as an entry point into the Coordinated Entry System and identify, engage, stabilize and house chronically homeless individuals and families. CORE teams will contact a minimum of 880 homeless individuals to provide basic needs supplies, counseling, benefits assistance, linkages to healthcare, shelter placement, and referrals and transportation to C.A.R.E. Centers. The County has been receiving funds from the City of Walnut Creek for the CORE program since January 2017.
On December 17, 2024, the Board of Supervisors approved Grant Agreement #29-604-9 with the City of Walnut Creek to receive CDBG and HSF funds in an amount not to exceed $20,000 for the operation of the CORE Program to provide homeless outreach services from July 1, 2024 through June 30, 2025.
