RECOMMENDATIONS:
APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Amendment Agreement #29-806-11 with the City of Concord, to pay the County an amount not to exceed $10,000 for the provision of homeless outreach services for the Coordinated Outreach, Referral and Engagement (CORE) Program for the period July 1, 2025 to June 30, 2026.

FISCAL IMPACT:
Approval of this Agreement will allow the County to receive an amount up to $10,000 for FY 2025-26, from the City of Concord to provide homeless outreach services. A 20% County match of $2,000 is required.

BACKGROUND:
CORE teams serve as an entry point into the County's coordinated entry system for unsheltered persons and work to locate, engage, stabilize and house chronically homeless individuals and families. County has been providing homeless outreach services to the City of Concord since July 2017.

On December 3, 2024 the Board of Supervisors approved Agreement #29-806-10 to receive funds in an amount of $12,500 from the City of Concord for the provision of homeless outreach services for the CORE Program which locates and engages homeless clients throughout Contra Costa County for the period July 1, 2024 through June 30, 2025.

Approval of Grant Agreement #29-806-11 will allow County to receive funds from the City of Concord and continue providing homeless outreach services, through June 30, 2026. This Agreement includes the County agreeing to defend, indemnify, save, and hold harmless the City of Concord for any claims arising out of County's performance under this Agreement. This Agreement is late due to prolonged negotiations between County and City of Concord.
