RECOMMENDATIONS:
APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Contract Amendment/Extension #25-012-50 with Homebase, a non-profit corporation, effective June 26, 2026, to amend Contract #25-012-48 (as amended by Contract Amendment/Extension #25-012-49), to increase the payment limit by $473,009, from $946,018 to an amount not to exceed $1,419,027, and to extend the termination date from June 30, 2026 to June 30, 2027, to provide additional consultation and technical assistance regarding continuum of care (CoC) planning and resource development to the Health Services Department.

FISCAL IMPACT:
Approval of this Contract Amendment/Extension will result in additional expenditures of up to $473,009 and will be funded by 87% U.S. Department of Housing and Urban Development (HUD) grant funds and 13% Medi-Cal Administrative Activities. (Rate increase)

BACKGROUND:
Homebase has been providing consultation and technical assistance to the Health Housing and Homeless Services Division with regard to HUD activities, including grant-writing services for County's McKinney-Vento application and County's CoC planning and resource development. The Contractor has been providing these services to the County since July 10, 1998. This Contract was approved by Health Services Personnel to ensure there is no conflict with labor relations. The Health, Housing and Homeless Services Division contract monitoring staff meet on a regular basis to ensure monitoring of performance measures set forth in the contract are upheld.
