RECOMMENDATIONS:
APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Agreement #29-805-6 with the Housing Successor to the Antioch Development Agency ("Agency"), a government agency, to pay the County an amount not to exceed $30,000 for the provision of homeless outreach services for the Coordinated Outreach, Referral and Engagement (CORE) Program, for the period from July 1, 2023 through June 30, 2024.

FISCAL IMPACT:
Approval of this Agreement will allow the County to receive an amount not to exceed $30,000 from the Agency to provide homeless outreach services. A County match is not required.

BACKGROUND:
The CORE Program locates and engages homeless residents throughout Contra Costa County. CORE teams serve as an entry point into the County's coordinated entry system for unsheltered persons and work to identify, locate, engage, stabilize and house chronically homeless individuals and families. The outreach teams will contact a minimum of 400 Antioch homeless individuals to provide basic needs supplies, counseling, benefits assistance, linkages to healthcare, shelter placement, and referrals and transportation to Coordinated Assessment Resource and Engagement (CARE) Centers.

Under Grant Agreement #29-805-6, County will receive funds from the Agency to provide CORE services for the period July 1, 2023 through June 30, 2024. This Agreement includes agreeing to defend, indemnify, and hold harmless the Agency for claims arising out of County's performance under this Agreement.
