RECOMMENDATIONS:
APPROVE and AUTHORIZE the Interim Health Services Director, or designee, to execute on behalf of the County Contract Amendment #25-012-49 with Homebase, a non-profit corporation, effective June 1, 2025, to amend Contract #25-012-48, to increase the payment limit by $449,509, from $496,509 to a new payment limit of $946,018 and to extend the termination date from June 30, 2025 to June 30, 2026 for additional consultation and technical assistance regarding continuum of care (CoC) planning and resource development to the Health, Housing and Homeless Services Division.

FISCAL IMPACT:
Approval of this Contract Amendment will result in additional annual expenditures of up to $449,509 and will be funded by 88% U.S. Housing and Urban Development (HUD) grant funds and 12% Medi-Cal Administrative, as budgeted by the Division for Fiscal Years 2024-2026. (No rate increase)

BACKGROUND:
Homebase has been providing consultation and technical assistance to the Health, Housing and Homeless Services Division with regard to HUD activities, including grant-writing services for County's McKinney-Vento application and County's CoC planning and resource development. The Contractor has been providing these services to the County since July 10, 1998. This Contract was approved by Health Services Personnel to ensure there is no conflict with labor relations. The Health, Housing and Homeless Services Division contract monitoring staff meet on a regular basis to ensure monitoring of performance measures set forth in the contract are upheld.
