RECOMMENDATIONS:
APPROVE and AUTHORIZE the Health Services Director, or designee, to execute, on behalf of the County Contract #72-272 with Meraki Health LLC, a limited liability company, in an amount not to exceed $1,428,000, to provide consultation and technical assistance services to the Contra Costa Health (CCH) Office of the Director and Public Health Services Divisions with regard to the CCH Transformation Initiative, for the period December 1, 2025 through November 30, 2028.

FISCAL IMPACT:
Approval of this Contract will result in contractual service expenditures of up to $1,428,000 over a three-year period and will be funded 100% by Future of Public Health funds.

BACKGROUND:
CCH is seeking to establish an Office of Strategic Initiatives to assist the Office of the Director and other divisions within CCH with the prioritization and execution of key projects aimed at addressing community health needs within Contra Costa County while also improving CCH's efficiency. Through funding for the Future of Public Health grant, the goal of this partnership is to assist the CCH Office of the Director with a coordinated process of identifying all existing and proposed improvement recommendations and opportunities, including recently developed transformation recommendations. The Contractor will help review and prioritize all recommendations to ensure alignment with CCH strategic goals, priorities and existing projects taking into consideration regulatory drivers, Medicaid/CalAIM and other factors within the federal fiscal landscape. This is a new Contractor who will provide these services and foster a deep understanding of the CCH's mission, values, and long-term objectives starting December 1, 2025.
