RECOMMENDATIONS:
APPROVE and AUTHORIZE the Interim Health Services Director, or designee, to execute on behalf of the County Agreement #78-080 with City of Antioch, to pay the County an amount not to exceed $629,836 to provide homeless outreach services for the Coordinated Outreach, Referral and Engagement (CORE) Program, for the period from March 1, 2025 through June 30, 2027.
FISCAL IMPACT:
Approval of this Agreement will allow the County to receive an amount not to exceed $629,836 from the City of Antioch to provide homeless outreach services through June 30, 2027. (No County match is required)
BACKGROUND:
The CORE program works collaboratively in small teams to engage and stabilize individuals experiencing homelessness to build rapport, identify appropriate resources, and link to housing programs that can end their housing crisis permanently. The CORE program will provide targeted street outreach and linkage services to individuals experiencing homelessness and will work collaboratively with Antioch Police Department, Healthcare for the Homeless, County's Behavioral Health Services Division, Coordinated Entry (CE) service providers, and other community-based programs providing services in the County. The Core program outreach services include housing navigation, connection to community resources, transitions into shelter or housing, and health education, with the goal of reducing homelessness in the City of Antioch.
Under new Agreement #78-080 County will receive funds to provide CORE homeless outreach services for the City of Antioch through June 30, 2027.
