Title: APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract amendment with Contra Costa Interfaith Transitional Housing, Inc. (dba Hope Solutions), to increase the payment limit by $2,097,307 to an amount not to exceed $4,282,894 to provide additional rapid rehousing and homeless prevention services to families enrolled in County’s Employment and Human Services Department’s CalWORKs program, and to provide 424 gift cards with a total value of $26,115 (207 Walmart gift cards up to $100 per card, 190 Target gift cards up to $25 per card, 26 Safeway gift cards up to $25 per card and one 1 Safeway gift card up to $15) for participating in the rapid rehousing program, with no change in the term ending June 30, 2026. RECOMMENDATIONS:
APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Contract Amendment #25-083-9 with Contra Costa Interfaith Transitional Housing, Inc., a non-profit corporation, to amend Contract #25-083-8, effective December 1, 2025, to increase the payment limit by $2,097,307, from $2,185,587 to a new payment limit not to exceed $4,282,894 for additional rapid rehousing and homeless prevention services to families enrolled in County's Employment and Human Services Department (EHSD) CalWORKs program and who are at risk of or currently experiencing homelessness and are eligible to receive services through the California Department of Social Services (CDSS) HousingWORKS! program. Contractor will purchase and provide two hundred seven (207) Walmart gift cards each with a one hundred dollar ($100) value, one hundred ninety (190) Target gift cards each with a twenty-five dollar ($25) value, twenty-six (26) Safeway gift cards each with a twenty-five dollar ($25) value and one (1) Safeway gift card with a fifteen dollar ($15) value, to eligible Participants, with no change in the term ending June 30, 2026.

FISCAL IMPACT:
Approval of this Contract Amendment will result in additional budgeted expenditures of up to $2,097,307 funded 100% by EHSD CalWORKs.

BACKGROUND:
This Contract meets the social needs of county's population by providing rapid rehousing and homeless prevention services to families who are at risk of, or currently experiencing, homelessness and recipients of EHSD's CalWORKs program and who are eligible to receive services through the CDSS HousingWORKS! program.
