RECOMMENDATIONS:
APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Contract #25-084-4 with Contra Costa Interfaith Transitional Housing, Inc. (dba Hope Solutions), a non-profit corporation, in an amount not to exceed $1,342,790 to provide rapid re-housing and homeless prevention services for homeless transition age youth (TAY) in Contra Costa County to achieve self-sufficiency and housing stability, for the period July 1, 2023 through June 30, 2025.


FISCAL IMPACT:
This contract will result in contractual service expenditures of up to $1,342,790 over a 2-year period and is funded by 93% Homeless Housing Assistance and Prevention Program (HHAPP) funds ($1,242,790) and 7% Housing and Homelessness Incentive Program (HHIP) funds ($100,000).


BACKGROUND:
This contract meets the social needs of county's population by providing rapid rehousing and homeless prevention services to the homeless TAY population in Contra Costa County. These services include case management which will assist participants with food assistance, substance use treatment, education and training, employment and benefits assistance, health care, mental health counseling, childcare, and credit repair. Rapid rehousing services will include providing financial assistance for security deposits, utility deposits/payments, moving costs, hotel/motel vouchers, paid shelter costs, housing application and credit/background check fees, partial, tiered or full rental subsidies, to TAY experiencing homelessness or who are at risk of becoming homeless.
