From:                                          Dr. Ori Tzvieli, Interim Health Services Director

Report Title:                     Annual Update of the Contra Costa Health Plan Quality Improvement and Health Equity Transformation Program

Recommendation of the County Administrator Recommendation of Board Committee

RECOMMENDATIONS:

ACCEPT the Contra Costa Health Plan (CCHP) 2024 Quality Program Evaluation, 2025 Program Description, and 2025 Work Plan of the Quality Improvement and Health Equity Transformation Program (QIHETP), as recommended by the CCHP Joint Conference Committee (JCC); and

AUTHORIZE and DIRECT the Chair of the Board of Supervisors to accept the above three documents, which have been included as attachments, and provide an authorizing signature on the accompanying signature page.

FISCAL IMPACT:

There is no fiscal impact for this action.

BACKGROUND:

To maintain compliance with the California Department of Health Care Services (DHCS) contract requirements for Medi-Cal managed care plans, CCHP must submit an annual program evaluation, program description, and work plan of the QIHETP, approved and signed by the governing body.

The above-mentioned documents were reviewed by the CCHP JCC on June 6, 2025. An accompanying staff report is included for reference.

CONSEQUENCE OF NEGATIVE ACTION:

If this action is not accepted, CCHP will not be in compliance with the DHCS contract. Noncompliance can lead to audit findings, fines, and, ultimately, loss of funding.
