2026 LMHA Program Grant Administration Update
LMHA Program achieved a high rate (90%) of timely mid-year report submissions from LMHA contractors. Ongoing technical assistance, regular communication, and clear reporting guidance contributed to improved compliance and provided valuable data to monitor program progress and outcomes. Below are a few examples of LMHA-funded programs that have made a significant difference in improving health outcomes and making an overall impact on communities.
LMHA Health Priorities: Partner Funded Outcome and Impact Report (6-months)
Healthcare Access
St. Vincent de Paul Free Medical Clinic provided 372 uninsured patients with 574 free medical visits and 1,770 free ancillary services such as labs, diagnostics, prescriptions, and medical supplies.
United Latino Voices (ULV) assisted 148 Bay Point residents with Medi-Cal and Basic Health Plan enrollment, renewals, and healthcare navigation, improving access to care for immigrant families.
Opportunity Junction's Health Career Pathway Program trained Certified Nursing Assistants (CNA) and Medical Assistants (MA), helping 64 participants gain the skills needed to enter high-demand healthcare jobs, thereby expanding the local healthcare workforce and improving community access to qualified providers.
Hijas del Campo's Community Health Worker supported farmworker families with Medi-Cal, Contra Costa Basic Health Plan, and CalFresh enrollment and renewals, improving access to health insurance and preventing lapses in coverage.
Mental Health
Family Justice Center - TeleHealth supported survivors with access to trauma and psychiatric telehealth care, including emergency evaluations and follow-up support.
