RECOMMENDATIONS:
APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Agreement #29-605-7 with the City of Pleasant Hill, to pay County an amount not to exceed $140,733 to provide homeless outreach services for the Coordinated Outreach, Referral and Engagement (CORE) Program for the period July 1 2024 through June 30, 2025.

FISCAL IMPACT:
Approval of this Agreement will allow the County to an amount not to exceed $140,733 from the City of Pleasant Hill for FY 2024-25 to provide homeless outreach services. No County match is required.

BACKGROUND:
The CORE team serves as an entry point into the County's Coordinated Entry System for unsheltered persons and work to locate, engage, stabilize and house chronically homeless individuals and families. The CORE Program provides homeless outreach services aimed at identifying homeless individuals, youth and families living without shelter and in locations not meant for human habitations. The County has been receiving funds from City of Pleasant Hill for the CORE program since March 2017.

On March 21, 2017, the Board of Supervisors approved Grant Agreement #29-605 with the City of Pleasant Hill to receive funds in an amount not to exceed $73,173 for the operation of the CORE Program from March 1, 2017 through June 30, 2018, which included agreeing to indemnify the City for claims arising out of the County's performance under the Agreement.

On September 11, 2018, the Board of Supervisors approved Grant Agreement #29-605-1 to increase the amount payable to the County by $81,197 to a new total of $154,370 and extend the termination from June 30, 2018 to June 30, 2019 to continue the operation of the CORE program.
