RECOMMENDATIONS:
APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Memorandum of Understanding (MOU) #78-106 with Contra Costa Community College District (4CD), to provide Coordinated Outreach Referral and Engagement (CORE) team to provide homeless outreach support for up to 100 transitional-age youth (TAY) who are homeless and referred to County by 4CD for the period January 1, 2025 through June 30, 2026.
FISCAL IMPACT:
None, this is a non-financial MOU.
BACKGROUND:
This MOU is being established to formalize a partnership between the County's Health, Housing and Homeless Services Division (H3) and 4CD to improve service delivery and data coordination with a focus on supporting up to 100 eligible TAY who are homeless and referred to County by 4CD. Services include providing a CORE TAY education team, to provide, targeted homeless outreach to students at each of the 4CD campuses, data collection, providing basic needs supplies, shelter placement, transportation, benefits assistance, and reporting on progress toward achieving programmatic goals. H3 and 4CD staff will meet regularly to review tasks performed, status of activities and address and resolve any issues that may arise.
Approval of MOU #78-106 will allow for the delivery of homeless outreach services and data coordination between County and 4CD to support homeless TAY through June 30, 2026. The MOU includes both parties agreeing to indemnify, defend and hold harmless the other for any claims arising from the performance under this MOU. This MOU is retroactive due to an administrative oversight by the Division.
