RECOMMENDATIONS:
APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Contract Amendment #25-135-1 with Contra Costa Interfaith Transitional Housing, Inc. (dba Hope Solutions), a non-profit corporation, effective February 1, 2026, to amend Contract #25-135, to increase the payment limit by $2,332,000, from $4,417,156 to an amount not to exceed $6,749,156, to provide additional rapid rehousing and homeless prevention services for individuals who are homeless or at risk of becoming homeless in Contra Costa County, with no change in the term ending June 30, 2027.

FISCAL IMPACT:
Approval of this Contract Amendment will result in additional expenditure of up to $2,332,000 and will remain funded 100% by the California Department of Housing and Community Development. (Rate increase)

BACKGROUND:
This Contract meets the social needs of County's population by providing rapid rehousing and homeless prevention services for adults who are at risk of or currently experiencing homelessness. These services include case management which will assist participants with food assistance, substance use treatment, education and training, employment and benefits assistance, health care, mental health counseling, childcare, and credit repair. Rapid rehousing services will include providing financial assistance for security deposits, utility deposits/payments, moving costs, hotel/motel vouchers, paid shelter costs, housing application and credit/background check fees, partial, tiered or full rental subsidies.
