RECOMMENDATIONS:

 

APPROVE and AUTHORIZE the Employment and Human Services Director, or designee, to execute a contract with Choice in Aging, in an amount not to exceed $389,828 to provide Case Management services for the period July 1, 2024 through June 30, 2026.

 

FISCAL IMPACT:

 

Approval of this contract will result in expenditures of up to $389,828: Funded by 100% Measure X Funds; of which $194,914 is budgeted in FY 24-25 and $194,914 will be budgeted in FY 25-26.

 

BACKGROUND:

 

The Employment and Human Services Department (EHSD) seeks to renew its Contract with Choice in Aging for countywide case management services with special attention to addressing priority populations, as defined in the Older Americans Act (OAA). This Contractor was selected through Contra Costa County’s procurement process, RFP#1911, for Case Management, Outreach and Transportation to provide services for older persons aged 60 and above and dependent adults 18 years and over residing in Contra Costa County.

These services are provided through a Measure X allocation authorized on November 16, 2021, item D.5, for Master Plan for Aging/Local Community Based Aging Services.

 

CHILDREN’S IMPACT STATEMENT:

This contract supports one of Contra Costa County’s community outcomes of the Children’s Report Card: (4) "Families that are Safe, Stable and Nurturing."

 

CONSEQUENCE OF NEGATIVE ACTION:

 

Should the proposed action not be approved by the Board of Supervisors, County will provide less case management services for older adults 60 years and over, and dependent adults 18 years and over.
