Report Title: Choice In Aging Title III-B Supportive Services
Recommendation: Recommendation of the County Administrator

RECOMMENDATIONS:
APPROVE and AUTHORIZE the Employment and Human Services Director, or designee, to execute a contract with Choice In Aging, in an amount not to exceed $296,982 to provide case management services for older persons for the period July 1, 2026, through June 30, 2028.
FISCAL IMPACT:
100% County Measure X Funds of which $148,491 is budgeted in FY 26-27 and $148,491 will be budgeted in FY 27-28.
BACKGROUND:
The Employment and Human Services Department seeks to contract with Choice In Aging for countywide case management services with special attention to addressing priority populations, as defined in the Older Americans Act. This Contractor was selected through Contra Costa County’s procurement process, Request for Proposals #1235, for Title III-B supportive services, Adult Protective Services case management, and Measure X to provide services for older persons aged 60 and above, regardless of income, residing in Contra Costa County.
These services are provided through a Measure X allocation authorized on November 16, 2021, item D.5, for Master Plan for Aging/Local Community Based Aging Services.
CHILDREN'S IMPACT STATEMENT:
This contract supports one of the community outcomes established in the Children’s Report Card: (4) "Families that are Safe, Stable and Nurturing."
CONSEQUENCE OF NEGATIVE ACTION:
Should the proposed action not be approved by the Board of Supervisors, County will provide fewer case management services for older adults 60 years and over, and dependent adults 18 years and over.
