RECOMMENDATIONS:
APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Contract Amendment #77-475-4 with JMJ Healthcare Inc. (dba JMJ Home Health Services), a corporation, effective December 1, 2025, to amend Contract # 77-475-2 (as amended by # 77-475-3), to increase the payment limit by $300,000 from $520,000 to a new payment limit of $820,000, for additional home health care services with no change in the term ending June 30, 2026.
FISCAL IMPACT:
Approval of this Contract Amendment will result in additional expenditures of up to $300,000 and will be funded 100% by Contra Costa Health Plan (CCHP) Enterprise Fund II revenues. (No rate increase)
BACKGROUND:
CCHP has an obligation to provide certain home health care services for its members under the terms of their Individual and Group Health Plan membership contracts with the County. Home health care services include but are not limited to intermittent skilled nursing, therapeutic and rehabilitative care in patients' residences. This Contractor has been a part of the CCHP Provider Network providing these services and fostering a deep understanding of the CCHP organizations mission, values, and long-term objectives since July 2022.
This Contract is entered into under and subject to the following legal authorities: California Government Code ?? 26227 and 31000; Health and Safety Code ? 1451. Health Services Personnel approved this Contract to ensure no conflicts with labor relations.
