RECOMMENDATIONS:
APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Grant Agreement #29-806-12 with the City of Concord, to pay the County an amount not to exceed $9,200 for County to provide homeless outreach services under the County’s Coordinated Outreach, Referral and Engagement (CORE) Program for the period July 1, 2026 to June 30, 2027.
FISCAL IMPACT:
Approval of this Agreement will allow the County to receive an amount up to $9,200 from the City of Concord, including a 15% County match of $1,380 from Medi-Cal Administrative Activities revenues, to provide homeless outreach services.
BACKGROUND:
The CORE Program identifies and initiates contact with homeless individuals living on the streets, assesses their housing and service needs, and facilitates connections to shelter, benefits, behavioral health and primary healthcare services throughout Contra Costa County. CORE teams serve as an entry point to County’s Coordinated Entry System for unsheltered persons and work to locate, engage, stabilize and house chronically homeless individuals and families. County has been providing homeless outreach services to the City of Concord since July 2017.
On March 3, 2026 the Board of Supervisors approved Agreement #29-806-11 to receive funds in an amount of $10,000 from the City of Concord for County to provide homeless outreach services for the CORE Program for the period July 1, 2025 through June 30, 2026.
Approval of Grant Agreement #29-806-12 will allow County to receive funds from the City of Concord and continue providing homeless outreach services through June 30, 2027.
