RECOMMENDATIONS:
APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Grant Agreement #28-882-8 with Sutter Bay Hospitals, to pay County an amount not to exceed $125,000 for the Coordinated Outreach, Referral and Engagement (C.O.R.E.) program to provide homeless outreach services for the period from January 1, 2024 through December 31, 2024.
FISCAL IMPACT:
Approval of this grant agreement will result in an amount of $125,000 for the county's C.O.R.E. program for homeless outreach. No County match required.
BACKGROUND:
The Concord Shelter in conjunction with the county's C.O.R.E. program is part of a comprehensive strategy to provide outreach and housing and services to the homeless in Contra Costa County. The C.O.R.E. program will dedicate three beds within the Concord Shelter for Sutter Bay Hospital patients who are homeless, identified to be too low acuity to access the Respite Shelter program, capable of self-care and are appropriate for placement in regular shelter programs. These homeless residents are allowed stay up to 120 days.
On April 18, 2023, the Board of Supervisors approved Grant Agreement #28-882-7 with Sutter Bay Hospitals in an amount not to exceed $100,000 to provide funding to support county's C.O.R.E. program to provide homeless outreach services for homeless residents for the period from January 1, 2023 through December 31, 2023.
Approval of Grant Agreement #28-882-8 will allow county to receive funds for County's C.O.R.E. program through December 31, 2024. The County is agreeing to defend, indemnify, and hold harmless the Contractor for claims arising out of County's performance under this Contract.
