RECOMMENDATIONS:
APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Contract #25-138 with Contra Costa Interfaith Transitional Housing, Inc. (dba Hope Solutions), a non-profit corporation, in an amount not to exceed $989,850, to provide rapid rehousing and homeless prevention services to families participating in the California Department of Social Services' (CDSS) Bringing Families Home (BFH) program who are at risk of or currently experiencing homelessness for the period July 1, 2026 through December 31, 2027.

FISCAL IMPACT:
Approval of this Contract will result in contractual expenditures of up to $989,850 over an 18-month period and will be funded 100% by CDSS BFH funds.

BACKGROUND:
This Contract meets the social needs of the County's population by providing rapid re-housing and homeless prevention services to eligible families receiving child welfare services through the Children and Family Services Bureau (CFS). Families receiving CFS services who are identified as at risk of or currently experiencing homelessness are referred by the Employment and Human Services Department (EHSD) to the Housing and Homeless Services Division (H3) to receive CDSS BFH program services. These services include, but are not limited to, financial assistance and wrap-around services such as case management, housing navigation and financial assistance for security deposits, utility payments, moving costs and rental assistance.

This Contract is entered into under and subject to the following legal authorities: California Government Code ? 26227 and 31000. This Contract was approved by Health Services Personnel to ensure there is no conflict with labor relations. The He...
