Referral History and Update
The Federal D. Glover Community Wellness Network represents a countywide investment strategy aimed at advancing community-driven solutions to improve health and wellness outcomes. The initial $1 million investment was stewarded by the East Bay Community Foundation, which served as the intermediary responsible for administering funds, supporting grantees, and coordinating implementation across community-based organizations (CBOs).
As TCAB continues to refine program categories, funding levels, and expectations for accountability and support, it is critical to ground decision-making in early implementation experience from the first cohort. This item aligns with TCAB's role in guiding equitable investment strategies, strengthening accountability structures, and ensuring that funding decisions are informed by community-centered implementation experience.
