RECOMMENDATIONS:
APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Contract Amendment #25-094-8 with Contra Costa Interfaith Transitional Housing, Inc. (dba Hope Solutions), a non-profit corporation, effective July 1, 2026, to amend Contract #25-094-5, to increase the payment limit by $1,350,000, from $3,652,516 to an amount not to exceed $5,002,516, to provide additional rapid rehousing and homeless prevention services for homeless adults and Transitional Age Youth (TAY) on probation with no change in the term ending June 30, 2027.

FISCAL IMPACT:
Approval of this Contract Amendment will result in additional expenditures of up to $1,350,000 and will be funded 100% by Probation Department funds.

BACKGROUND:
This Contract meets the social needs of the County's population by providing rapid rehousing and homeless prevention services for homeless adults and TAY on probation. These services include case management and housing navigation services which will assist participants with food assistance, substance use treatment, education and training, employment and benefits assistance, health care, mental health counseling, childcare, and credit repair. Rapid rehousing services will include providing financial assistance for security deposits, utility deposits/payments, moving costs, hotel/motel vouchers, paid shelter costs, housing application and credit/background check fees, partial, tiered or full rental subsidies. This Contractor has been fostering a deep understanding of the County's organization, mission, values, and long-term objectives since July 2021.
