Report Title: Contract # 40-520-1 (Aging and Adult Services) with Meals on Wheels Diablo Region

RECOMMENDATIONS:
APPROVE and AUTHORIZE the Employment and Human Services Director or designee, to execute a contract with Meals on Wheels Diablo Region, a non-profit corporation, in an amount not to exceed $210,172, to provide case management services for County residents aged sixty or older, or persons with disabilities aged eighteen and older for the period July 1, 2024, through June 30, 2026.
FISCAL IMPACT:
Approval of this contract will result in expenditures of up to $210,172: 100% County Measure X Funding, of which 50% ($105,086) is budgeted in FY 24-25 and 50% ($105,086) will be budgeted in FY 25-26.
BACKGROUND:
Employment and Human Services Department (EHSD), Aging and Adult Services Bureau, has contracted with Meals on Wheels Diablo Region for the provision of case management services for County residents aged sixty or older, or persons with disabilities aged eighteen and older since July 1, 2023.
Services include, but are not limited to needs assessment, care plan development, and authorizing and coordinating services among providers to assist in meeting the needs of the county participants in the Aging and Adult Services Program. The original Contract was awarded in 2023 through Request for Proposal #1191 for FY 2023-24. The renewal of this Contract will allow Contractor to continue providing services through June 30, 2026.
CHILDREN'S IMPACT STATEMENT:
This contract supports item number (4) of Contra Costa County's community outcomes of the Children's Report Card: "Families that are Safe, Stable and Nurturing."
