Report Title: Contract Amendment #77-430-6 with Jiva Health, Inc

RECOMMENDATIONS:
APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Contract Amendment #77-430-6 with Jiva Health, Inc, a professional corporation, effective May 1, 2026, to amend Contract #77-430-3 (as amended by Contract Amendments #77-430-4 and #77-430-5), to add neurology services in addition to primary care physician (PCP), medical specialist services including endocrinology, diabetes, allergy, sleep, podiatry, rheumatology, telehealth and non-physician services including durable medical equipment (DME) services for Contra Costa Health Plan (CCHP) members and County recipients, with no change in the payment limit of $10,000,000 or term ending December 31, 2027.
FISCAL IMPACT:
Approval of this Contract Amendment will not result in additional expenditures and will remain funded 100% by Contra Costa Health Plan (CCHP) Enterprise Fund II revenues. (No rate increase)
BACKGROUND:
CCHP has an obligation to provide certain PCP services, medical specialist services including endocrinology, diabetes, allergy, sleep, podiatry, rheumatology, neurology, telehealth and non-physician services including DME services for its members under the terms of their Individual and Group Health Plan membership Contracts with the County. This Contractor has been a part of the CCHP Provider Network providing these services and fostering a deep understanding of the CCHP organizations mission, values, and long-term objectives since January 1, 2022.
This Contract is entered into under and subject to the following legal authorities: California Government Code ?? 26227 and 31000; Health and Safety Code ? 1451. Health Services Personnel approved this contract to ensure no conflicts with labor relations. Contractor currently ...
