CONSIDER approval of the June 18, 2025 Health Care for the Homeless Co-Applicant  
Governing Board meeting minutes. PROVIDE direction to staff as needed.

RECEIVE an update on Health Care for the Homeless services from Nurse Program  
Manager Mia Fairbanks.

RECEIVE an update on HCH program productivity from Gabriella Quintana (HCH QI Team).  
Data Review and UDS vs HCH Comparison

DISCUSS board member recruitment and training.  
Continue recruitment  
6. RECEIVE an update on the Consumer Advisory Board (CAB) from Breanna Lingenfelter (HCH  
QI Team).  
Kathleen has decided to step down as co-chair and member for now for personal reasons but still wants  
to stay involved without impacting quorum.  
Dianna will take her place with Donnie connecting with Alameda CAB and plan to attend their meeting  
next month.  
Erik and Bobby want to become members so we hope to interview/vote them in soon.  
CAB finalized a survey for site visits and hope to collect lots of feedback/introduce new people to HCH.  
Two new guests at our last meeting and general consensus from all is that they feel comfortable going to  
HCH team because they don’t feel judged. Some stated their first interaction was Outreach team  
approaching them in their camps offering help.  
“It all started from there..now I have housing, two years clean and still have the business card Harukah  
first gave me” (keeps it in his wallet)

Sept 12-15 stand down on the delta – 13th is community day  
Hope solutions – probation east county trying to bridge the gap with an event in late august. Resource  
fair.  
Mashal - Housing readiness training (HOPE Solutions)  
We are also happy to conduct disability etiquette trainings and a bit on what we do here at ILR –  
Christine Burke
